RN Responsibilities
- Provide comprehensive case management, assessments, and ongoing support for members’ health, medical, and safety needs
- Develop, review, and maintain Plans of Care and Health Care Records
- Offer training and technical assistance on health-related matters to caregivers and UCP employees
- Consistent monitoring and documentation of caseload
- Conduct initial eligibility assessments and monitor hospitalizations of members
- Perform quality assurance reviews as required
- Attend necessary meetings with internal and external stakeholders
