Valley Medical Group is seeking an energetic, personable and motivated full time Outreach Clinical Clerk for our Greenfield Health Center. The hours for this role are 8:00AM-5:00PM. Position: Outreach Clinical Clerk Location: Greenfield, MA Responsibilities: As the Outreach Clinical Clerk you will perform outreach to all Valley Medical Group patients according to Valley Medical Group Outreach Guidelines, maintaining a simple, coordinated, effective and patient centered process for outreach. As the Outreach Clerk you will serve as a liaison between patient’s and medical support staff. Duties for this role include but not limited to updating patients charts with correct diagnosis and scheduling all services due according to the tickler system and quality management data.
