Join our team of competent and caring professionals as a Clinical Care Coordinator at the CBFS (Community Based Flexible Support) program at Behavioral Health Network! We are dedicated to supporting and encouraging the pursuit of the development of individuals’ autonomy, self-esteem, and involvement in their own care. Clinical Care Coordinators work closely with adults with serious mental health issues and structure their care by providing preliminary screenings, critical service recommendations, and interventions and referrals. Connecting with the individuals’ nursing, psychiatric, and peer support staff as well as outside providers and family members, Clinical Care Coordinators complete comprehensive assessments and create individualized care plans to support their development and independence. They are committed to the implementation of the care plans, following up with individuals and their providers to ensure that the plan’s goals are being addressed and strived for.

At the CBFS (Community Based Flexible Services) program at Behavioral Health Network, we aim to help adults living with a severe and persistent mental health diagnosis live independently and flourish in their community. CBFS is a voluntary community outreach program funded by the Department of Mental Health and serves the communities of Ware, Palmer, and Belchertown, MA. Behavioral Health Network is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. Requirements:
Clinical Care Coordinators at BHN’s CBFS program have the following qualities:
• Experience working with adults suffering from mental challenges.
• Strong clinical skills (preferred).
• Master's Degree in the behavioral health-related field (required).
• Clinical Licensure eligible.
• Clean driving record and reliable car (required).
