Role Description

This is a part-time hybrid role for a Licensed Practical Nurse. The LPN’s day-to-day tasks will include providing direct patient care, monitoring patient conditions, documenting patient progress, and collaborating with other healthcare professionals to improve patient outcomes. The LPN will be working in the Hampshire County and be available 24-32 hours a week with the opportunity to grow into a full-time position with benefits. Will be responsible for one weekend a month on call.

Qualifications

• Active LPN License
• 2 years of experience working with elders
• Experience in long-term care or a related field
• Excellent communication and interpersonal skills
• Ability to work flexible hours and adapt to changes in patient assignments
• Willingness to work in a fast-paced environment
• Attention to detail and strong organizational skills
• Experience working with electronic medical records is a plus
• Willingness to receive training in areas such as dementia care and fall prevention
• Ability to be on-call
