Become a part of our team of competent and compassionate professionals as an Access Navigator in the Certified Community and Behavioral Health (CCBHC) program at Behavioral Health Network! Access Navigators have a unique role in patient care. As an Access Navigator, you will build working relationships and provide in the moment interventions and referrals to support patients while they learn to self-navigate the health care system. Access Navigators work across the BHN service array to support patient transitions and access to care. They work in collaboration with the primary care provider and all members of the health care team. They are responsible for direct and indirect patient care for the patient population. They serve as patient advocate and assist in identification and improvement of service delivery, especially the facets of care that facilitate better access. This position requires understanding of community resources and providers, both internal and external to BHN.

Behavioral Health Network (BHN) is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. We are dedicated to offering high quality, affordable and culturally appropriate care to people of all ages and income levels in our constituent communities

The Certified Community and Behavioral Health (CCBHC) program is an innovative model of care, working directly with primary care and pediatric providers to address the whole health needs of the families and individuals in our culturally diverse community. Our staff includes psychologists, social workers, counselors, recovery coaches and care coordinators. The CCBHC program is person-centered, evidence-based, and focused on collaborative goal setting to address unmet medical, behavioral mental and social needs.
Requirements:
Access Navigators at Behavioral Health Network have the following qualities:
•High school diploma; Bachelor’s degree preferred.
•Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management (required).
•Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components (required).
•Strong verbal and written communication skills (required).
•Familiarity with Internet, word processing, and email systems (required).
•At least 1 year experience working with target population.
•At least 1 year working in a team-based setting (preferred).
•Community Health Worker (CHW) certification (preferred).
•Bilingual English and Spanish (preferred).
