These are individuals who may be reluctant to engage in services or who are coping with behavioral health challenges. A strengths-based approach is used to work in partnership with the client to assess needs, create a plan for well-being, expand support systems, and address underlying issues. In addition, this position will provide care-coordination for private pay clients. Must have experience doing home-based casework with adults with behavioral health challenges. Bachelor degree in social work or related field and two years experience required. Masters degree preferred.
