The Access Navigation Supervisor is central to increasing access and eliminating gaps/redundancy in treatment for clients identified as in need. They supervise Access Navigators and patient flow, and track and monitor the quality of connection to care functions. The Access Navigation Department conducts in-depth research into the services already in place or available to the individual and will ensure that a care coordinator and a care team are in place to monitor and support the client successfully connecting with care. Our staff includes psychologists, social workers, counselors, recovery coaches and care coordinators. The CCBHC program is person-centered, evidence-based, and focused on collaborative goal setting to address unmet medical, behavioral mental and social needs. Requirements: Access Navigation Supervisors at Behavioral Health Network have the following qualities: •Bachelor's Degree is required. •Master’s Degree in related mental health field preferred. Licensure at independent practice level preferred (LICSW, LMHC, PhD) •Bilingual English and Spanish (required). •Being able to do Outreach-responding to an individual’s needs in community settings (required) •Experience in the human services or related fields (required). •Experience working in a team-based setting (required). •Supervisory and/or managerial experience. •Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management (required). •Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components (required). •Knowledge of community based resources (required). •Strong verbal and written communication skills (required). •Familiarity with Internet, word processing, and email systems (required). •Community Health Worker (CHW) certification (preferred).
