SEEKING LICENSED PRACTICAL NURSE (Admin)
To work in our residential programs throughout Hampden Counties

We are seeking a licensed practical nurse to work with our developmentally disabled and brain injured clients in their residential programs. This is a full time position and will be based out of Holyoke, MA but will travel to programs throughout Hampden County.

As an LPN, you will:

• General nursing care at a cluster of residential, supported living and outreach programs
• Vital signs, physical and weight tracking assessments as needed.
• Occasional skilled nursing care (wound care, catheter care, etc.)
• Arranging for VNA services when indicated.
• Case management and hospital visits in the event of a resident getting hospitalized.
• Attend occasional DDS and/ or ABI meetings; establish ongoing communication with these funders, particularly the nurse representative.


You are someone who has:

• Current MA LPN License
• Minimum 3 years nursing experience
• Strong working knowledge of the Medication Administration Program
• Experience working with adult special needs population
• Perform quarterly audits and enter results into our data base.
• Some rotating on-call responsibilities (via telephone only).
• Valid Driver’s License and acceptable Motor Vehicle Registration
• Provide results to the Program Director, Director of Operations and Nurse Director.
• Some rotating on-call responsibilities (via telephone only).
• Staff education/ attendance at staff meetings as needed to provide education on a variety of topics
• Attendance at monthly supervision with the Nurse Director.
• Valid Driver’s License and acceptable Motor Vehicle Registration
