Description: | Case Manager-Full Time
LifePath seeks a full time case manager. Ideal candidate will be comfortable working independently and as part of an interdisciplinary team providing case management and administrative support for the Home Care program. The position serves consumers in Franklin county and the North Quabbin area.
Ideal candidate will have excellent:

* Advocacy skills * Ability to establish positive relations
* Flexibility * Multi-tasking skills
A minimum of two years experience providing case management supports to elders and/or persons with disabilities. Bachelor's degree preferred. A commitment to supporting people to live safely and independently in the community. Knowledge and experience in person-centered planning. Successful candidate will have reliable transportation and ability to frequently travel throughout assigned areas.
