Housing Stabilization Case Manager

Interested in joining our team of dedicated professionals? Way Finders is currently seeking a full-time Housing Stabilization Case Manager. The Housing Stabilization Case Manager provides efficient and empathetic stabilization services to homeless families placed out of shelters.

The candidate will work in a hybrid-remote capacity but must live within commutable distance to our main office in Springfield, MA.  In-office work and in-person meetings are occasionally expected.

Responsibilities include:

  • Conduct assessments of client housing needs and other barriers that affect stabilization including financial, legal, mental health, substance abuse, employment, health care and housekeeping; triage as needed for maximum housing stability
  • Review and complete monthly stabilization touchpoints; develop and implement goals that address identified needs and maximize self-sufficiency
  • Coordinate support services, initiate referrals, and provide advocacy, crisis intervention, and supportive counseling to clients
  • Develop partnerships with internal and external service providers to ensure comprehensive and seamless service delivery
  • Educate and assist families on financial literacy and obtaining needed resources
  • Enter and maintain accurate data in a timely fashion for use in tracking family progress on established goals
  • Conduct monthly contacts with clients via home visits, phone, text, email and/or virtual platforms; monitor family progress in meeting goals during stabilization
  • Maintain complete and accurate virtual case files in a timely manner
  • Assist in the mediation of issues between clients and owners/landlords
  • Assist client with struggles that directly impact tenancy preservation
  • Establish and maintain strong, collaborative relationships with staff (including across departments), owners, landlords, community groups, funding sources, constituents, and stakeholders

Requirements include:

  • Bachelor’s degree in relevant subject matter such as social services or public administration; commensurate experience may be a substitute for a degree
  • 3 years’ experience in housing or social services case management; similar or related experience may be substituted
  • Ability to assess priorities, organize, and operate in a flexible manner while retaining a high level of attention-to-detail
  • Excellent interpersonal skills with the ability to serve diverse populations and work appropriately with confidential information
  • Demonstrated ability with Microsoft Office applications, and comfort learning new technologies
  • Bi-lingual (Spanish/English) verbal and written skills preferred but not required
  • Valid driver’s license with acceptable driving record; reliable vehicle
