Essential Job Functions

The primary duties of this position include but are not limited to the following.  The employee will be accountable for and will be evaluated on the successful, and professional achievements of these responsibilities.  The work is expected to be completed within the assigned schedule, hourly commitment, and budget.   Compliance is expected with Pioneer Valley Hospice & Palliative Care's Policies and Procedures, agency and professional standards of practice, ethics and to facilitate the mission of Pioneer Valley Hospice & Palliative Care.

Essential Functions

  • Formulate and utilize spiritual assessments, interventions, outcomes and care plans in order to contribute effectively to the well-being of the person receiving care.
  • Provide spiritual care that respects diversity and differences including, but not limited to culture, gender, sexual orientation and spiritual/religious practices.
  • Review initial spiritual assessment of newly admitted patients and families at weekly IDT meetings, as requested; respond to referrals and requests for services within standards.
  • Contact by telephone all referred patients/families/caregivers within five days of admission unless requested sooner by patient/family (with exception of those who clearly decline spiritual support).
  • Serve as liaison between patients/families and local clergy as needed and as directed by plan of care.
  • Coordinate with local clergy spiritual and religious rituals (prayer, baptism, etc.) as requested for patients/families.
  • Ability to represent and articulate Integritus HealthCare mission, vision and strategies in developing, implementing, and coordinating the SC program.
  • Other duties as assigned by the Clinical Director.

Key Interdisciplinary Team Responsibilities

  • Attend weekly Interdisciplinary Team meetings, as requested and report on status of persons being seen, facilitate identification of spiritual issues in all patients/families discussed.
  • Serve as consultant to Team on spiritual issues, needs and religious practices, at IDT meetings or as needed.
  • Participate in agency programs to educate the community about hospice philosophy and care, particularly to include outreach to the local faith community, as requested.
  • Serve as a spiritual support resource to staff members and volunteers related to their own on-going work as caregivers with patients and families experiencing illness, dying, and grieving processes.
  • Maintain quality and productivity standards and participate in the measurement of these standards.
  • Demonstrate advocacy for those we serve and facilitate the mission of the agency.

Key Administrative Expectations

  • Complete documentation in electronic medical record according to policy. Complete and submit for filing any appropriate clinical documents according to policy.
  • Attend meetings as directed including meeting with supervisor, required in-services and training, monthly staff meetings.
  • Meet individual and program expectations and goals as identified through performance appraisals, the annual plan or by supervisor.
  • Efficiently manage agency resources and funds.
  • Employee may be asked to perform other duties as assigned by the clinical services coordinator.
