Position Summary: This is a full-time, 40-hour per week position. The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of the Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient outcomes.
Pay range: $35.00 - 39.04, based on years of experience.
Essential Job Functions
- Provides direct patient care within the parameters of the current nursing license.
- Implements current nursing practice by following the patient assessment and approved plan of care, which was initiated by the RN Case Manager.
- Complete, maintain, and submit accurate and relevant clinical notes regardingthe patient’s condition and care. Record pain and symptom management changes and/or outcomes as appropriate.
- Point of Service documentation is an expectation of all clinical personnel for quality care, accuracy, and communication standards. The clinician is expected to appropriately document at Point of Service, have timely synchronization of patient data & effectively communicate with the Patient Care Team.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
- Participates in the coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members.
- Uses and prepares equipment and supplies appropriately, adhering to aseptic technique.
- Provides appropriate pain and symptom management. Evaluates the patient’s response to treatments and medications.
- Participates in personal and professional growth and development.
- Participates in the organization’s quality management program.
- Participates in on-call schedule, including evening, night, holiday, and weekend rotation to ensure the agency's ability to provide 24-hour coverage.
- Other duties as assigned.
Qualifications
(Minimum qualifications will be considered required unless specifically stated otherwise)
Experience:
- Minimum of one (1) to two (2) years' experience in health care. Three (3) years experience in health care preferred.
- Experience in a hospice care is preferred.
- Experience in acute care in an institutional setting preferred.
- Experience in homecare preferred.
- Understands hospice philosophy, and issues of death/dying.
Education and Training:
(Acquired through formal education, outside study, training on jobs of lesser degree, or by any combination of these. May be expressed in terms of formal educational equivalents):
- Graduate of an accredited Practical Nurse or Vocational Nursing program.
License, Certification & Registration:
- Possesses and maintains current CPR Certification.
- Current Massachusetts licensure: LPN/LVN
- Valid driver’s license

Working Conditions:
- May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying repair.
- May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA exposure category:
- OSHA exposure category: Category I
- Category I ? Position includes tasks that involve exposure to blood, body fluids, tissues.
- Category II ?Position includes tasks that involve no exposure to blood, body fluids, tissues, but employment may require unplanned Category I tasks.
- Category III ? Position includes tasks that involve no exposure to blood, body fluids, tissues; would not be required to perform Category I tasks.
