Registered Nurse Position Overview:
The Registered Nurse (RN) is an integral part of the Adult Family Care (AFC) and Family Support Multidisciplinary Teams for UCP of Western Massachusetts. This position will work in collaboration with the Clinical Nurse Supervisor and Director of Pioneer Valley Services to ensure the healthcare needs of the members in the programs are adequately supported. This position will work with agency participants, family members, and the multidisciplinary professional teams to ensure plans of care are developed and followed, provide ongoing support and maintain paperwork for all members enrolled.
This position requires travel to North Adams, MA once weekly for training. Once training is complete travel to North Adams will be 1-2 days per month as needed.
Registered Nurse Responsibilities:
- Oversee healthcare services, document medical needs, and participate in support plans and case conferences.
- Advocate for optimal health and medical needs of assigned program members.
- Coordinate medical services with primary care physicians and manage medication orders.
- Ensure compliance with Department of Developmental Services (DDS) clinical standards.
- Manage confidential medical records and obtain necessary reports and treatment approvals.
- Provide case management, health advice, and ongoing support for medical needs.
- Develop, supervise, and maintain medical policies, procedures, and member assessments.
- Oversee AFC caregivers, conduct training, review caregiver logs, and ensure quality care through regular visits.
